The mark consists of two ovals with a white band between them; the outer oval is defined by a border consisting of two green ovals with a yellow oval between them; the inner oval creating the white band is green; in the white band, in green letters, are the words "BOARD OF CERTIFICATION IN", running around the top from approximately 9 o'clock to 3 o'clock; in the bottom of the white band, also in green, are the words "FAMILY MEDICINE"; also in the white band, separating the upper and lower words are two green dots, one on each side; the interior oval is yellow with a green staff, outlined in white, with a green snake, also outlined in white, wrapped around it forming a caduceus; at the bottom of the yellow oval in green letters are the words "SINCE 1985"; to the left of the caduceus, in white outlined in green are the letters "BC" and to the right of the caduceus, also in white outlined in green are the letters "FM". Outside the oval at the bottom curving around it, in black, are the words "A MEMBER BOARD OF THE AMERICAN BOARD OF PHYSICIAN SPECIALTIES".

The certification mark, as used or intended to be used by persons authorized by the certifier, certifies or is intended to certify that the persons certified by the Board of Certification in Family Medicine have met the following eligibility requirements that include: · Completing an ACGME- or AOA-accredited residency in family medicine, or a family medicine residency accredited by the College of Family Physicians of Canada (CFPC) · Submitting a current curriculum vitae that includes medical school, degrees earned, and work experience, in chronological order · Submitting a documented report of at least ten family medicine cases for which the physician had the lead management role. The case reports must be no older than twelve months from the date the candidate's application for certification is submitted to the ABPS. · Successful completion of a computer-based written exam that consists of 200 multiple choice questions administered over a four-hour session.
