[ Providing seniors with referrals to commercial service providers for services not provided by Fellowship Village, in the nature of referrals for home repair and maintenance, landscaping, lawn care, snow removal and automobile repair ]

Motor Vehicle Delivery Services, namely, providing food and drug delivery, home delivered meals and personal transportation; clinical social worker services, namely, assessing transportation needs and coordinating motor transportation needs for seniors

Providing assisted living facilities; residential retirement housing and continuing care retirement communities for seniors; supplying meals to residents of assisted living and retirement communities; elder day care; clinical social worker services, namely, assessing, coordinating and monitoring housing services

Providing nursing homes and skilled nursing facilities, including subacute on-site medical services in nursing homes and retirement communities; medical services provided by physicians, nurse practitioners, nurses and related professionals; medical services related to dementia and memory loss; hospice care; palliative care; physical, occupational and speech therapy programs; medical and nutritional counseling, namely, illness management in the nature of providing limited in-home personal medical services for individuals including checking vital signs, medication compliance and scheduling doctor appointments and providing nutritional analysis and recommendations and ensuring compliance with individual hygiene; medication management, namely, preparation of and dispensing of medications; dietician services; nursing, dentistry and podiatry services; all inclusive program services in the nature of delivering needed medical and supportive care services, namely, providing physical and occupational therapy and providing recreational therapies in the nature of guided activities such as musical entertainment, celebrations, social clubs, arts and crafts, games, exercise, dancing and gardening to increase, mobility, stimulate cognitive functioning and increase affiliation with others and self-expression, and nutritional counseling; medical services provided by a program physician familiar with the history, needs and preferences of each senior participant; home health care; hospital and nursing home care; clinical social worker services, namely, conducting an assessment program for senior patients receiving various medical, rehabilitative and therapeutic services for the purposes of guiding treatment and assessing program effectiveness

Providing geriatric care management services in the nature of coordination of necessary services and personal care for seniors, namely, the coordination of home inspections, annual physical examinations, home health aide services, housekeeping services, elder companion services, emergency response system services, meals delivery and provision, adult day care, lifestyle and wellness program, dog walking services, and transportation for providing food, drugs, meals and personal transportation; all inclusive program services in the nature of delivering social work and personal care services in the nature of assisting with bathing, toileting, transferring, dressing and grooming and other aspects of personal hygiene, and assistance with activities of daily living; and respite care in the nature of short term or temporary non-medical personal care of the senior to provide relief to the regular caregiver, such as a family member, as necessary; clinical social worker services, namely, coordinating medical, rehabilitative and therapeutic treatment and evaluating treatment effectiveness for senior patients
